PROGRAM FOR PEOPLE WITH TRAUMATIC BRAIN INJURIES 18+

Thank you for your interest in the NeuroResource Facilitation Program (NRFP), a grant-funded program through the Pennsylvania Department of Health.

The purpose of the program is to increase awareness of, and access to, resources and services for Pennsylvania residents with traumatic brain injuries (TBIs), age 18 and older.

A NeuroResource facilitator meets with each person and his or her family members to explain the services available in their local area,
* provide them with written materials describing services of interest and the eligibility criteria (if any),
and help them with identifying and accessing the services they need.*

▪ The facilitators stay in contact with them for six months, to provide any additional help that may be needed to select and receive services.

▪ The goal of the NRFP is to help individuals with TBIs maximize their health, capabilities, and well-being, and to achieve a better quality of life by connecting them with services and supports.

▪ There is no cost to the individual or the family.

The NeuroResource Facilitation Program (NRFP) is currently serving individuals in the Southwest, South Central, Southeast, and Northeast regions of the state.

By 2020, we will be serving the entire state.

Examples of resources we have assisted individuals to access include transportation providers, brain injury support groups,
centers for independent living, and advocacy organizations.

We have also assisted individuals to apply for services or support through Community Health Choices (CHC), the Office of Vocational Rehabilitation (OVR), and Social Security Administration (SSA).
