IDD/ODP TRANSITION
If my child is receiving Early Intervention (EI) services do the services automatically transfer to IDD services when my child reaches the age of 3 years old?
NO! Upon your child’s third birthday EI will conduct a re-evaluation to determine whether your child still exhibits a delay. If no delay still exists then services will end. If a delay still exists then services will be transferred to the appropriate organization or agency in your county that provides services.
2. When do we begin transition planning and what does that look like?
Transition planning for the youth with an Individual Education Plan (IEP) begins at age 14 with specific goals and activities added into the IEP. The youth may be able to stay in school until they are 21 years of age. This will be determined by your IEP team.
When the youth is 16 years of age the Service Coordinator should complete or update a PUNS form (see explanation for what a PUNS is) for transition planning purposes.
Transition concerns may include, but are not limited to:
Life Skills, Supported Living Arrangements
Pre-Vocational Programs, Competitive Employment
Transportation
Other important things to consider during the transition years:
Apply or re-apply for Social Security/Medical Assistance benefits at age 18.
Contact Private Insurance for continued coverage.
Consider changing physicians from pediatric to adult.
Register with the Office of Rehabilitation and Vocational Services (OVR) within the last two years BEFORE graduation.
Your youth’s school should help facilitate this need.
